$1,000 Diabetes Expense Grant Guidelines

The Jones Foundation has teamed up with Team Schnak Strong Fund to provide a $1,000 Diabetes Expense Grant to eligible applicants in Coffey, Lyon and Osage Counties to help offset expenses associated with the treatment of Childhood Type 1 Diabetes. This grant is payable in the amount of $1,000 per applicant, per year up to the age of 21 and may be re-­applied for annually.

NOTE: If you so choose, you may also apply for the Children’s Medical Expense Grant

Eligibility Requirements for the $1,000 Diabetes Expense Grant:

1. The applicant must be a US Citizen under the age of 21 and a current resident, or the dependent of a current resident of Coffey, Lyon, or Osage County and who has continuously resided in one (or a combination) of these three counties for a minimum of one year.

2. An applicant who moves to one of the three counties and attends a post-secondary education institution before fulfilling the one-year residency requirement is not eligible. Attendance at an education institution in the three-county area does not alone establish residency.
